Post-Course Survey
Family Seven Foundations (F7F) would like to thank you for your participation in this program and for taking part in this survey. Participation is appreciated so that we can continue to meet client expectations and improve overall outcomes for families. However, participation in the survey is voluntary.
Some important things to know about the survey:
The information that you are sharing is protected.
The information is compiled and used as part of F7F’s Continuous Quality Improvement (CQI) efforts and initiatives.
CQI focuses on supporting you and future participants in reaching optimal outcomes.
CQI monitors the programs fidelity to content, engagement of students, assimilation of content by the families, as well as the integration of the newest research and best practices into the curriculum.
It is anticipated that this survey will take no longer than 15 minutes. If assistance is needed to complete this form, your Family Education Facilitator is available to help with both the reading and comprehension of the survey. However, answers must be provided solely by the participants.
FSF would like to again express our gratitude for your help in assisting us to continually improve our program. We are all working together to strengthen and preserve the family unit.

If more than one parent is participating, each parent will need to fill out a survey.
Each participant will verbally acknowledge their understanding of the following, and will be allowed an opportunity to ask any clarifying questions:
I understand that this survey is voluntary, and that I will neither be compensated, nor penalized for taking this survey. I understand that by participating in this survey I am contributing to the gathering of information that may benefit my personal success, and/or the future success of other families. I also understand that all information is kept confidential. Names for the purpose of authentication are kept at a separate location from the surveys. Each family name is associated with a number that remains with the survey responses to add an additional level of security in maintaining confidentiality.
